My body twitches and shakes occasionally. My obsessive-compulsive disorder is becoming more challenging to manage. Every day, I feel the urge to die. I have the urge to do it whenever I can. Unfortunately, I've started to injure myself. I always stab myself with a compass. Now my left hand is covered in bloodstains. I'm concerned and scared that I might not be able to control my impulses. I'm so afraid that I can't even bear to look at it. I think about how to die every day, but I don't want to die. It's not that I'm afraid, it's that I'm afraid of leaving my family behind. Could I be sick?

If I might inquire further, could you please tell me how long this has been going on? I'm asking because I'd like to understand your situation better.

If you feel anxious, you might like to try this breathing method: Inhale, say "xu" softly, exhale, say "ke" softly, inhale, say "si" softly, exhale, say "chui" softly, inhale, say "hu" softly, exhale, say "xi" softly. Repeat this six-breath method for ten minutes, placing your awareness at the nostrils and paying attention to your breathing, feeling the sensation in the nasal cavity as air enters and exits the nose.

If it is comfortable for you, you may now either sit or lie down. If you prefer, you may close your eyes. Take a moment to imagine yourself bathing in a hot spring or in the sun. Allow yourself to feel warm all over and completely relaxed. Now, if it is comfortable for you, you may begin to relax your dominant hand. You may notice it becoming heavier and heavier. It may even become warmer. You may then relax the other hand and both legs in the same way.

Once the limbs have relaxed and become warm, we can move on to relaxing the abdomen. The abdomen is soft, slowly warming up and becoming warmer. This is good. Next, we can focus our attention on the forehead and feel the tension there. A cool breeze is blowing, and the forehead is becoming cooler.
